Unfortunately, the term "upscale" has been so over- used in this hobby that the connotations associated with it, at least in my mind, are not positive at all. Luxury seems appropriate for the experience you are describing.
In order to answer your questions, I have mentally retraced my hobby experience (which, admittedly, is mostly overseas - Singapore, Germany, Brazil, etc) to try to compare the over/ under experience at or above the price-point of $300 US that you mentioned.
What makes a luxury companion in my mind:
1) The amenities - much as you described, a nice clean incall with easy parking. Men's hair and skin care products available for the pre-play shower (think about the care packages better airlines give first-class passengers on an international flight). Perhaps there is wine/liquor available for ONE pure-play gtky drink
2) The look - the companion is clean and elegant, i.e. has OBVIOUSLY taken the time to put effort in her appearance. Made up, hair done, dressed well, lingerie, in decent shape, etc. There CANNOT be a let down when the door opens.
3) The personality - well spoken, easy conversationalist. Has some knowledge of a lot of different topics, simply to keep conversations going (silent moments are death to social interactions).
4) Confident - both in conversation and between the sheets. Note that I don't refer specifically to "classy," another very-much overused term in this hobby. One of my very best "luxury" hobby experiences included the companion, while still very much clothed, leaning in and whispering in my ear "I'm going to fuck your brains out," then proceeding to do EXACTLY that.
5) Attitude - people talk about GFE, this is more like a "new GF" experience. Not only has sexual empathy and a real desire to please the client in a deeply satisfying way, but has a service mentality. Luxury, for me at least, requires little effort.
6) TCB - easy TCB and good communication! I can't stress this enough. A PROFESSIONAL communicates in such a way that there is no uncertainty for the client.
7) Service/menu - this is last for a reason as menu is usually one of the key selection criteria for providers, i.e. if you've been selected, the hobbyist already agrees with your menu - but the menu should be clear and consistent. For me personally, at this price point for a "GFE Luxury" companion, "no" is almost never in her vocabulary, within her menu, of course (obvious and notable exceptions include Greek, BBFS and SM). I won't see a companion at that price point that doesn't include BBBJ/CIM, for example.
All that being said, all it really means is that at the end of the session, the client thinks that it was a luxury experience. That's where your empathy is important.
Finally, I think this may be difficult to pull off in a 4/5 star hotel. The overhead and logistics may make it unprofitable - but that's a different discussion for a different time.
