    Guys, let's talk ED

    This is not to bash anyone and it's not speaking on any individuals.
    I am not a real doctor so don't be trying to file a grievance against me. Lol

    Sharing (good) information is caring ....
    I'm sure most of you guys have discussed this topic
    possibly in your man cave. But all of the guys are not in the man cave I don't think ?

    What works, what doesn't?
    Remedies? Recommendations?

    Have any of you guys tried the Hormone Pellets?
    I know the Pellets are one remedy that has worked
    for guys and they love the results.
    I have recommended some of my clients to try out the Hormone Pellets and
    they have all gotten back with me stating 100% positive results.
    However the Hormone Pellet remedy is expensive I believe, but worth the end results.

    What remedies/pills/creams would you recommend staying away from?
    I know some of you make a last minute stop and grab those gas station pills,
    obviously some of you put your trust in those. Maybe the gas station pills are good?
    Are they regulated?

    Is the prescribed little blue pill the fix it all and the most trusted?
    Is it ok to sip wine or any other cocktail if taking the blue pill? I would think not
    but I'm a female and don't really know and I don't think the blue pill is for females.

    Physical exercise and prescription medication like Viagra and Cialis are safe ways to avoid having ED issues during a session. Alcohol can make ED worse and can lower blood pressure to a dangerous level when combined with ED drugs.
    Staying hydrated helps avoid low blood pressure.

    Cialis works for me. I'm scared of those gas station pills - no idea what's in them. I do take a mild BP med, but no issue with the C (at least not yet, anyway).

    Best drugs for ED are any of the "afil" range of drugs such as sildenafil (brand Viagra), tadalafil (brand Cialis), vardenafil (brand Levitra and Staxyn), etc.... These are all vasodilators. Basically they help dilate blood vessels and increase blood flow to certain areas of the body (specifically smooth muscle tissue - like the penis). I'll give a little science and physiology lesson here b/c I'm so fond of it!

    The penile tissue of the male body, the corpus cavernosa, fills with blood normally upon arousal. For some men who have lower "T" levels (possible) as well as age, genetics, taking certain medicines, etc... they may experience difficulty in getting an erection.


    So how do these vasodilators work (the "afil" drugs)? I'll explain it in roughly 7 "parts" to try to break it down into manageable chunks.

    1. Normally during sexual stimulation the body produces NO (nitric oxide) from nerve endings and endothelial cells in the corpus cavernosa. An enzyme called guanylate cyclase then converts guanosine triphosphate (GTP) into cyclic guanosine monophosphate (cGMP).

    2. This is the important part. cGMP is what allows the muscle tissue to relax, causing an influx of blood and results in an erection. Men who have trouble getting an erection usually produce too little NO (nitric oxide).

    Why does that cause a problem? And how do vasodilators like Viagra help? Good question

    3. cGMP is then hydrolysed into inactive "GMP" (not "cGMP"). This action occurs via an enzyme called PDE5 which is most notably found in the corpus cavernosa (penis). The problem with men who don't have enough NO is that the PDE5 enzyme is breaking down the cGMP into inactive GMP too quickly, not allowing a full hard erection to ever materialize (or only very briefly).

    4. So, what Viagra, and all ED drugs do, is bind to the PDE5 enzyme, blocking its functioning (at least partially).

    5. This means that since PDE5 can't hydrolyse cGMP fast enough to inactive GMP they maintain an erection.
    That whole thing about the 2, 3, 4hr+ erections? That's b/c the ED drug is working too well and is blocking the PDE5 enzyme severely such that the penis can't hydrolyse cGMP (which is allowing blood flow into the penis by relaxing smooth muscle tissue) into GMP. This means they get "stuck" with a raging boner. It's very rare but it happens.

    6. Normally an ED drug works well enough to allow blocking of the PDE5 enzyme hydrolsysis of cGMP into inactive GMP only so much that the rate of cGMP available is great enough to maintain a completely solid erection but an orgasm, or the removal of stimulation, which will also cause NO (nitric oxide) release to decrease, as well as GTP binding, which then results in a drop of creating cGMP, will cause the erection to subside.

    7. The former item #6 happens as the PDE5 enzyme which are available to do work (not all PDE5 are bound by the ED drug - that's what causes those super long erections) are breaking down the existing cGMP into inactive GMP faster than the inflow of new cGMP. As a result the erection subsides and the penis goes back to a flaccid state.

    This is how all of the ED drugs work. In a nutshell.

    I don't know anyone that has tried the TRT (testerone replacement therapy) hormone pellets. My dad used the creams and tried the injection. I've also tried the creams and gels myself.

    Personally I would advise anyone to just use a compound cream mixture (5 or 10% usually) or one the gels. Just go see your doc and get some labs and find out if you need to boost your T level at all. It can DEFINITELY help with ED. It can be so profound for some people according to various research that they don't even need ED drugs anymore. TRT is VERY effective. But do not over do it. It can have bad side effects if you let the levels get too high (such as low sperm counts, acne, anger/rage, prostate issues, etc....). The low sperm count seems weird but the reason is that excess T in the body is converted to estradiol (estrogen). You can even get male breast tissue (yep - it's called gynecomastia). That's a common issue with bodybuilders who overtake anabolic steroids.

    TRT injections can have a rising and falling effect. Strong in the beginning and then wear off toward the end of the injection cycle (usually 2 weeks). This can cause mood changes, mental clarity issues, physical issues with your workouts, etc...

    The pellets appear to have the possibility of causing infection. In addition if you needed to change doses it wouldn't be so easy since you have these "pellets" inserted under your skin. They could also rupture or come out. I don't like the idea. I'd stick with creams or gels.

    The gas station stuff and most of the "T" therapy junk at GNC (except for very few) are total crap. Complete waste of money. Most of them do jack and you might as well take a sugar pill. This is true for the testosterone boosters and most of the erection enhancers. There might be some anecdotal data from individuals but there is nothing conclusive in any valid study to support the claims of those products.

    Nope, they are not FDA approved.

    They all work well ~P. It depends on the man and his physiology regarding the PDE5 inhibitors, nitric oxide production, cGMP hydrolysis, etc.... Certain ones will work better for some guys but basically they all do the same things I outlined above.

    Yes, some wine or a few drinks is not a problem but you don't want to get wasted b/c alcohol is a CNS depressant and can simply make getting an erection, or arousal at all, more difficult anyway.

    Yes, they work for females. Male and female physiology are actually quite analogous barring some major organ differences such as the uterus, ovaries, testes, prostate. Studies have shown that the PDE5 enzyme also exists in the vaginal, clitoral, and labial smooth muscle tissues just as it does in men. It's not technically approved by the FDA for use by women but it is allowed as an option for doctors in "OFF LABEL USE". Basically there is enough data to support it that the FDA allows the doctors to prescribe what they think will work best for their patient (man or woman). There is plenty of research to support the fact that it increases blood flow in the vaginal area and clitoris which ends up allowing for an easier orgasm. The clitoris is a direct analogue to the penis.

    I believe there is a new drug that is also coming on the market, just approved by the FDA, for use by women. However, like I said, many women have reported positive effects from taking Viagra, Cialis, etc.... It's also not surprising at all given the way it functions.

    I agree totally with your first statement and about the hydrating, absolutely The alcohol part needs to be discussed in a qualified perspective. It is not always bad. You are right that it *can* be bad. Alcohol itself actually raises BP. However, yes, certain ED medicines, b/c of the way all ED medicines work, by increasing blood flow and dilation of blood vessels, can result in a lowering of blood pressure. There is no issue at all with 1-3, or even 4, drinks for any ED drug. The main issue with going further and entering in heavy drinking or binge drinking territory is that alcohol itself is a CNS (central nervous system) depressant so it can make the ability to get an erection even more difficult.

    I’ve been taking Cialis/Tadalafil for the past three years. But I get it from a doctor’s prescription. I’m not going to order shit from some foreign website, and I damn sure won’t use some shit from a gas station bathroom.

    i get my sildenafil from Maple Leaf Meds and there have not been any problems. The cost is $.60/pill since Viagra is out of patent. I can't swing $10-20/pill

    Yea that's for more severe ED issues. It's called alprostadil. It's another type of vasodilator, very powerful, injected directly into the penis.

    Older men are obviously going be the typical users. Sometimes artery blockage, diabetes, and various other ailments or physiologic anomalies can cause the inability to produce an erection naturally (even with something like Viagra or Cialis). The injection can provide an alternative for those men.

    It's a bit odd that he doesn't/didn't orgasm. There is no reason why a man at any age, if he can attain an erection, shouldn't be able to mentally & physically climax. If they can't it's usually serious arterial damage, or diabetic neuropathic anomalies, certain cancers, prostate surgeries, etc... that may cause an inability to "ejaculate" (however, ejaculating is different than orgasm or climax)

    Outside of that it's usually something in their mind, causing a condition that's known as Male Orgasmic Disorder. We all have to learn how to orgasm and our body will produce the ejaculate as part of that process.

    Some men who masturbated as young teens, or very young adulthood, develop psychological issues such that they can't orgasm/climax with exception to manual hand stimulation or, sometimes, oral stimulation. They may have extreme trouble or find it impossible to climax in the vagina. This is very typically a mental issue and it's believed that it has something to do with conditioning themselves to have this behavior in their youth through excessive or abnormal masturbating (or even the use of devices such that a vagina, for example, doesn't "feel" right).

    After hearing the story of your client, I would wonder and even postulate, that maybe he does climax/orgasm. He just cannot produce an ejaculate. Orgasm/climax and ejaculation are two completely separate things. Maybe he had prostate surgery or some form of cancer or has other arterial blockage? Or maybe he produces what is know as a "retrograde ejaculation"? That's a condition where climax occurs but the ejaculation empties into the bladder instead of out the penis. It's not harmful but usually an indication of infertility. Also known as a dry ejaculation.

    In that case it wouldn't be strange if he produced little to zero semen; but climax still occurs.

    Also, any man who has dry ejaculations on subsequent orgasms but otherwise has normal ejaculations simply has an "empty tank" LOL There is no semen left in the prostate & no sperm in the seminal vesicles. It was all exhausted in the prior ejaculation(s) (usually 1-2 without at least a 30-60min interval). This is why I say it's important to realize that orgasm/climax & ejaculation are two completely separate things. A man can climax or orgasm with zero ejaculate. This is very possible. So maybe he did climax, but didn't ejaculate?

    On the subject of climax vs ejaculation, there is a huge difference. I don’t know if it’s due to age or pharmaceuticals, but I don’t ejaculate much at all any more. Some BPH medicines I used to take caused the total elimination of ejaculate. When it first occurred I was a bit freaked out about it, but now I just tell women that I’m easy cleanup.

    I can still have some amazing orgasms. It just takes me some time to get there. I’m sure ~P’s spankings would help.

    OK, back on topic, I was surprised to discover that sildenafil was pretty effective for me. Now that it’s so inexpensive, a no-brainer.
