I agree completely with Anna. You cannot guarantee someone an orgasm regardless of the amount of time scheduled. You can do your very best, you can even add some time, maybe even fairly considerable time if you have it, and you really like them, but you cannot guarantee them an orgasm.

Same thing if you goto a tattoo artist and get a tattoo that afterwards you decide you aren't completely happy with, even though the artist gave you pictures, showed you in a mirror what it would look like, etc... They did the work, used their equipment, spent their time, and you have to pay.
Yes, some artists might give you a partial refund, maybe. In order to pacify a customer and to not hurt business, maybe. Same with the medical doc situation. However that's up to them. If they take their time, use their supplies, and do whatever work requested, you have to pay; and there are [usually] no refunds in such cases.
If you paid a tennis coach to teach your son competitive level tennis in order to get on an elite H.S. team but your son doesn't make the cut do you think the coach should refund the lessons?? None of these things are her fault. If you didn't like her look your should have said something upfront. All the other things are on the guy, if she provides passionate intimacy and he can't cum that's on him.
He shouldn't expect any refund, depending on a few circumstances, like maybe there was no chemistry and she realizes it too and that she couldn't be very intimate or passionate, so she "might" offer some sort of partial discount or refund. Up to her.

Otherwise I cannot imagine expecting a refund. I have never ever expected a refund or discount. Yes, there have been times I couldn't cum over the various years. Another interesting question, I feel this question has alot of variables. I agree with anna and mathguy here that there are no guarantees in this business or many other business in fact. i feel like there are factors in play; hence many YMMV are mentioned.
1. The amount of time booked: i believe, personally, that QV or half hour shouldnt be guarantee anything. But those are too short of a time for a provider to do everything they can to make you feel comfortable and then get you off assuming without a hitch. However, for an hour or more sessions, i do believe there should be a 1 "pop" but no guarantee for multiple. If you offer the option to, then thats completely depends on how the session goes. There isnt a playbook and here so every session is different. But assuming everything is normal, (guy has no medical condition, can get hard, didnt jack off like 15 times before coming to the session), for an hour, i do believe 1 "pop" would be so call "guarantee" as long as both parties are doing their parts.
2. Service level: as i mention on my previous point, both parties doing their part. it does mean that both the provider and client is trying to achieve that. If you book and hour session and you expect the provider to ride you for an hour to get you off, thats not gonna happen. Provider can get tired and it can be physically exhausting just being on top the entire time. But at the same time, (this is somewhat from experience) provider cant just lay there or have the back turn and just tell the client to get on with it. I had experience one time where the provider just say "im all dicked out" after a couple minutes and stop providing service. So, yes if both parties are trying and doing their best to achieve this 1 "pop" and it doesn't come i think that would lie in the clients fault.
But, as i and others mention, there are not guarantees. However, if you believe that the session is somehow your fault that the 1 "pop" didnt happen in an hour long session. Maybe as a curtesy to extend a little time to try or give some discount for the next visit. I dont think refund is a thing in this business and i think if anyone ask or expects a refund is probably not gonna have a great time since the mood is already ruin. But i only said the discount next visit as to only if you believe session didnt go well due to a fault on your part.
