I just want to let you know that if I haven't seen You within the past year and only saw you 2-3 yrs ago I will not be giving you a reference anymore. You will need to have a recent visit with me within a years time frame if it's longer than a year I can not give you a reference Please don't be offended but ppl change as it has been brought to my attention and 2-3 yrs not seeing someone puts my memory to the test
to recall our appts.
Providers deserve to have recent references about there potential appts , like I said ppl can change so I'm order to give you a good reference I need a recent appointment to go by
