I dated a couple of ladies that were squirters over the years. It’s not nearly as fun in civilian life. You have to change the sheets every single time you have sex. Every. Single. Time. After the ego boost is gone it’s really just a messy distraction. Both my girlfriends said the squirt was different than an orgasm...felt good but not nearly as good or intense as cumming. I'll weigh in here b/c of my own personal experiences as well as my educational background in bio-sciences and physiology/anatomy.
I have often said that squirting is peeing. It's not exactly "peeing" but most studies (one was referenced above by BrianneBanks iirc?) do contain ultrasounds of pre & post orgasm bladder contents. In one particular smaller study with 7 women I believe it was 6 of the 7 (it might have been all 7 - I don't recall exactly - I'll have to find it) who all had urine seepage (i.e. their bladder emptied to varying degrees). This was even the case with the women who absolutely believed they were not "peeing".
The deal with "squirting" and why it's so terribly controversial is that even today we don't have conclusive evidence of all the mechanisms in play and/or exactly what the physiological response is in females. To complicate matters there is massive data suggesting that genetics play a huge role b/c some women do not have the particular vestibular glands believed to be responsible for the fluid (or they don't have nearly as many as others do). I also have certain empirical evidence of my own which leads me heavily to believe it's based largely on those genetic factors. These glands are also called the Skenes gland, they are paraurethral gland(s) on the anterior vaginal wall.
Squirting originates from research that identified the g-spot in females. This produces intense orgasms and is a small nub of tissues just behind the pubic bone inside an inch or two and up (this is why it's a bit hard to reach at times). The intensity of these orgasms lead to women expelling fluid as they lose control and orgasm. It's not that they never happened before it's just like all things as the information age and research went on the public became vastly more aware of women who exhibited this "phenomenon". You do not *have* to have g-spot stim to have an expulsatory orgasm but it typically helps for most women.
The issue about whether it's pee or not is largely b/c of studies that show via ultrasound that there is bladder leakage that occurs post orgasm. Compounding this is the absolutely absurd expelling of massive amounts of fluid in porn where the women are most undoubtedly peeing. It's quite obvious b/c it's clearly yellow(ish) colored. Porn has a fascination with it and this made things even more bizarre. Much like porn skews young mens ideas of how large a penis should be or how much men ejaculate (not even realizing that most of it is fake, camera angles, tiny women, camera/mirror automatically makes one look "bigger" - quite considerably actually). Most of those men are very average sized (or just above). Almost none of them are beyond 7.5in and many are the typical average 5.5-6in (length). Same deal with squirting. All the sudden it was this crazy sexy thing for a woman to spray liquid everywhere And so the era of female porn stars peeing on men became the norm and they call it "squirting" (after all, it's not as if you can smell it via the film, or even see the coloring that well). Some people are clever enough to hide the color by packing the woman's vagina with lotions, soaps, creams, etc... which cause this absurd creamy mess to pour out
MY OWN EMPIRICAL EVIDENCE (WHAT I CAN RECALL ANYWAY):
In my life I have been with about 4 women that could squirt. Real squirting. Real orgasms. One girl smelled awful, it was obvious that she mostly pee'd. In fact, I question if that's all she did upon orgasm. It was yellow, it smelled foul, and it was pee. Period. Plain and simple.
Another one (my ex-wife) could squirt. She found out with me. She was 24-25 at the time. She was super emabarrassed b/c she didn't know what had happened. She was a looker and very sexual and had been with about 20+ men at that young point in her life but she had never squirted. In the beginning of our sexual relationship she believed it was another "fluke" like the one guy some years earlier. I told her no I thought her body just needed to rejuvenate and likely produce more fluid again. That was my hypothesis. Lo and behold when we went 3-4+ days or a week without sex, particularly a full week+, and then did it again, she could gush like a geyser, no odors, no detectable urine, just lots of clear (or milky) fluid all over.
This leads me to believe that it is indeed a fact that some women simply have many more of the Skenes ducts and some of them produce (and refill with time) a large quantity of fluid relative their size than for other women. For other women it causes bladder leakage to a larger degree b/c of their unique physiology, lack of as many Skene's glands, and/or they have sex nearer the time the bladder is full. Naturally the loss of control of muscles during orgasm (a woman doesn't have conscious control of those muscles during a real orgasm) causes more seepage of the urine in the bladder and the Skene's glands are emptied quickly. This is why some women will have that pungent aroma mixed in; such as I mentioned with my ex-wife. She also loved to have sex in the morning when she felt the urge to pee b/c this gave her the best orgasm. I've noticed this quality with 2 different women.
All of this leads me to the inevitable conclusion that "squirting" is largely dependent on a few factors (and is possibly different for each woman to one degree or another):
a) bladder contents
b) absence or over-abundance of Skene's paraurethral glands
c) psychological factors allowing them to fully embrace an orgasm and be truly uninhibited with their partner
d) g-spot stimulation
All of this above having been said, I want to be clear that it is my position that most of the women in porn, as well as many women who claim to be able to do this feat "on demand", are doing nothing more than peeing on their partner.
However, I also believe there exist expulsatory orgasms in women which contain differing levels of urine, sometimes a lot, sometimes tons, sometimes very little, and even sometimes none. I've seen it firsthand. I simply can't reconcile the volume of fluid from porn films, or that some women claim to do, along with what I've personally seen in my private sexual life, as well as the fact that anatomically it doesn't make sense.
At the very least there is a recovery window (as I mentioned above with the last girl in my experiences above). Even in men there is a recovery period. You cannot just produce semen fluid (no, not sperm, however that too, no way) constantly. The body has to recover and produce more. Semen is produced in the prostate and makes up most of what people think is "sperm" - about 95% of it - the rest which is sperm are created in the testes and release during ejaculation to mix with the PSA fluid, or, semen (only about 3-5% of semen is live sperm). Anyways, same with females. The Skene's is homologous to the male prostate. And, again, on some women they cannot even find these paraurethral ducts, they don't exist, or they are so, so small they can't produce any noticeable fluid.

A: It depends. Mostly on the individual female.
Verdict: It all depends but it does usually contain some bladder content. Sometimes it's virtually all urine. Sometimes it's a mixture of a PSA like fluid with elements of urine present. Sometimes it's a near odorless PSA fluid from the paraurethral glands with zero (or very little) urine (or very mild urine which has no noticeable odor). Most studies do show at least some bladder content lost post orgasm.
Notes/Caveats:
*** 98-99% of adult videos are women peeing on demand, expelling urine with excessive force using their muscles like a "reverse kegel" in order to make it spray or shoot. A real g-spot or squirting orgasm doesn't "spray" or "shoot" in such an exaggerated manner. It mostly just leaks/gushes out (but not with a force that can propel it).
*** Realize that urine does not have to smell. It can be very very mild with not much odor if any. Only certain conditions cause the smell (dehydration is a good one). Urea breaks down to ammonia easily & so certain urine can be foul smelling but it's also possible to be virtually odorless which is likely why some people believe there is absolutely no urine present in fluid that results from g-spot orgasms.

This part is highly questionable. A number of studies agree that the fluid is indeed from the urethra. That doesn't mean it's pee though, of course.
However, it does makes the whole issue of, "some women, or sometimes, it can have a weird odor, pungency, etc...", all the more reasonable and gives a plausible explanation. For example, it could simply be urine left in the urinary tract that mixes with fluid from the gland.
It's not always two holes or ducts.

The more noticeable thing though is that the 7 women in a separate sex study (within the same abstract) had a range of 4 to 7 paraurethral ducts. It was only those women who could produce any noticeable mucosa or PSA like fluid other than urine upon orgasm:

These are some very interesting articles and most of the information gives us plenty of reason to see why some women can do it, some women can't, and sometimes it has a urine smell, sometimes it doesn't have any or much at all, and still sometimes it's virtually entirely urine.
Similarly to the g-spot some women simply appear not to contain certain tissues in the clitoral network that would allow that type of orgasm. Much of the spongy tissue known as the g-spot is the same type as found in the male penis (regarding g-spot, not Skenes duct). For women who are stimulated at the g-spot area we can even see the tissue respond directly to male erectile drugs such as Viagra. Pretty crazy hmm? It's not all that weird though when we consider that we all start the same in the womb. The clitoris and what would become the vagina/vulva grows into a penis and testes as the male fetus develops.
